You can contact your insurance provider to ask for your plan's specific coverage and costs by

calling the number on the back of your insurance card and asking the following questions:

1) Do my benefits cover psychotherapy (CPT codes 90791 & 90837) with Dana Bowerbank (NPI: 1356925853)?
